Upon receiving an inquiry
for our services, an At Home Care
coordinator will make an appointment to visit your home. The appointment
is set at a time convenient for you, your family and other interested
parties.
Initial Meeting
During the initial meeting,
an assessment is performed to identify your needs, requirements
and overall home situation. Our coordinator will explain At
Home Care's services and will suggest the level of service
deemed most appropriate. This meeting leads to a mutually agreed
upon Plan of Care.
Plan of Care
The Plan of Care
is a document outlining the specific care required including: schedules,
client preferences, and special instructions. The At
Home Care coordinator works with the client and family
to achieve a complete level of satisfaction. As your situation changes,
the plan is modified to reflect those needs. The Plan of Care
provides a central focus for all parties involved with your care.
Introducing
the Caregiver
Once the plan of care is
accepted and a service agreement is completed, we make every effort
to match you with the best caregiver based upon personalities, skill
sets, expertise, interests and attitudes. Our goal is to create
the best, most comprehensive in home care possible.
Continued Follow-Up
After we begin care, an
At Home Care coordinator
continues to stay in touch with you and your caregiver to ensure
the best possible service is being provided. In addition, a daily
care log is kept by the caregiver for the family's review. If at
any time you or your family is not satisfied with our services a
new caregiver will be assigned. We at, At
Home Care, want to exceed the client's expectations and
ensure their quality of life.
